Crash investigators are appealing for witnesses to come forward after two women, one heavily pregnant with twins, were killed in a car accident in Sydney's west last night.

The multi-vehicle collision around 7:40pm also left two men critically injured.

Police said a Mazda 3 travelling south on the Northern Road, near Glenmore Parkway at Orchard Hills, collided with a Nissan Tiida travelling in the opposite direction.

The 17-year-old girl driving the Nissan and her rear-seat passenger, a 23-year-old woman, who was heavily pregnant, died at the scene.

The crash also claimed the lives of the pregnant woman's unborn twins.

The 25-year-old male front-seat passenger in the Nissan was taken to Westmead Hospital in a critical condition.

A 29-year-old man and sole occupant of the southbound Mazda was trapped for a short time but he was eventually removed and taken to Westmead Hospital in a critical condition.

He remains under police guard.

"One of the men has limb injuries, a pelvic injury and a head injury," NSW Ambulance Acting Inspector Joe Ibrahim said.

"The other one also has a head injury and has been put into an induced coma by our medical team."

Two other vehicles suffered minor damage as they took evasive action to avoid the crash when the Nissan was forced backwards.

Police said they were investigating what caused the crash and a report was being prepared for the coroner.
