Shortly before 11pm, officers attempted to stop the car on the Wakehurst Parkway in North Narrabeen. Police said officers had received information regarding the driver's welfare.
While police were following the car, the vehicle crashed into a tree and caught alight.
The driver, who was alone in his vehicle at the time, died at the scene.

Earlier on Monday night in a separate crash, a cyclist died in Double Bay when his bike collided with a car.
Emergency services were called to New South Head Road about 6.30pm, and found the male cyclist had died at the scene.
